Term | Definition |

Anonymity | The patient's right to have their health information collected in a way that it cannot be traced back to them |
Audit Trail | A record that traces employee activity within the EHR (Includes unsuccessful attempts to view unauthorized screens) |
Authentication (Password Entry, Digital Fingerprint/ Voice Signature, or SmartCard) | The method of determining whether the person trying to access a given EHR/Network has the authorization to do so |
Authorization | A document giving a covered entity permission to view PHI for secondary use purposes, payment, health care operations or to disclose PHI to a 3rd Party Specified by the patient |
Breach | An impermissable or disclosure under the HIPAA Privacy Rule that compromises the security or privacy of the PHI |
Business Associate | a person or entity that performs certain functions or activities that involve the use or disclosure of PHI on behalf of the covered entity |
Confidentiality (The duty of maintaining health information privacy) | The obligation of medical professionals to keep a patient's information limited to only those that are authorized to gain it |
Consent | Permission given to a covered entity for uses and disclosures of PHI for treatment, payment and healthcare operations |
Consumer Reporting Agency | An agency regulated by the FTC under the Fair Credit Resporting Act that sells or cooperatively exchanges consumer credit information AND history |
Covered Entities | Health care providers, covered entities, and healthcare clearinghouses that transmit health information electronically |
Disclosure | Giving access to, releasing, or transferring information to a person or entity |
Ethics | Rules and standards of coduct that govern professional behavior and come from our shared understanding of morality |
Laws | Formal enforceable rules and policies based on community standards of conduct |
Minimum Neccesary Standard (MNS) | A key provision of the HIPAA Privacy Rule requiring that covered entities limit unnecessary or innappropriate access to and disclosure of PHI. Disclosures should include only the minimum necessary amount of information to accomplish a given purpose |
Off-Label Indication (Applies specifically to medication) | A use for a prescription drug other than what it was initally intended for (FDA approved use) |
Password | A set of characters and/or numbers used to prevent unauthorized access to or disclosure of patient information contained within secure electronic files |
Privacy | The patient's freedom to determine when, how much, and under what circumstances their medical information may be disclosed |
Protected Health Information (PHI) | Individually identifiable health information that is stored, maintained or kept electronically |
Safeguards (Specified by the HIPAA Security Rule) | Measures taken to prevent interence with compuer network operations, and to avert security breaches involving the unauthorized use, disclosure, modification, erasure, or destruction of PHI |
Screen Saver | A program that displays animation or image on the screen if input (such as pressing a key)is not recieved for a given time period |
Secondary Use (Applies specifically to health information) | A use of health information that is not directly related to patient care |
