I.O.D line of duty
Reporting the injury

When supervisors become aware of an injury on duty | supervisors of injured employees are required to prepare the "Supervisor's Investigation Report of Employee Injury or Illness" (P–1063) |
and the Florida Department of Labor and Employment Security | "FIRST REPORT OF INJURY OR ILLNESS" forms within twenty–four (24) hours |
The "Supervisor's Investigation Report of Employee Injury or Illness" form P–1063 must be completed | and distributed within twenty–four (24) hours after the injury, even if no medical treatment is required |
The immediate supervisor of an injured employee is required to prepare the necessary | Florida Department of Labor and Employment Security multi–copy form "FIRST REPORT OF INJURY OR ILLNESS" |
Do not list any JSO facility (P.M.B., Sub–Station, P.D.F, etc.) as | the employee's home address |
The City of Jacksonville Worker's Compensation Section is required to send employees | their rights under Workers' Compensation Rules, as well as, notices of doctor's appointments, and other documents to the employee's home addresses |
Absence due to necessary treatment for such injury will be documented | on the Roll Call as "I.O.D |
via an E.I.S | leave request |
This will be completed by the Roll Call Supervisor or designee | if the employee is not able to complete the I.O.D. leave request |
Members carried on Roll Calls as I.O.D. are expected to be found | at their respective homes, physician's offices, hospitals, or enroute to one of those locations |
If the employee has not returned to duty after 30 calendar days | the Personnel Division will transfer the employee to an IOD roll call via EIS |
