Level 1 No Response: Total Assistance Complete absence of observable change in behavior when presented visual, auditory, tactile, proprioceptive, vestibular or painful stimuli
Level 2 Generalized Response: Total Assistance Responses to pain or repeated stimuli. Responses are gross movements, may be delayed
Level 3 Localized Response: Total Assistance demonstrated appropriate responses to stimuli. withdrawal-to pain, blink-to vision, etc.
Level 4 Confused/Agitated: Max Assist Alert and heightened stated of activity. Mood swings, aggressive behavior.
Level 5 Confused, Inappropriate, Non-Agitated: Max Assist alert, not agitated but may wander randomly or with a vague intention of going home.
Level 6 Confused, Appropriate: Moderate Assistance Inconsistently oriented to person, time and place Able to attend to highly familiar tasks in non-distracting environment for 30 minutes with moderate redirection
Level 7 Automatic, Appropriate: Minimal Assist for ADL Consistently oriented to person/place, in highly familiar environments. Moderate assist for time Able to attend to highly familiar tasks in a non-distraction environment for at least 30 minutes
Level 8 Purposeful, Appropriate: Stand-By Assistance Consistently oriented to person, place and time Independently attends to and completes familiar tasks for 1 hour in distracting environments Uses assistive memory devices to recall daily schedule
Level 9 Purposeful, Appropriate: Stand-By Assistance on Request Independently shifts back and forth between tasks and completes them accurately for at least two consecutive hours Uses assistive memory devices to recall daily schedule,
Level 10 Purposeful, Appropriate: Mod Independent Able to handle multiple tasks simultaneously in all environments but may require periodic breaks Able to independently procure, create and maintain own assistive memory devices
