nursing diagnosis related to drug therapy deficient knowledge: drug therapy regimen, dificient knowledge: safe and effective self-administration, risk for injury related to adverse drug effects, noncompliance: overuse, noncompliance: underuse
expected outcomes should be stated in terms of patient behavior
examples of expected outcomes pt will: receive/take drugs as prescribed, experience relief of symptoms, avoid preventable adverse drug effects, self administer safely and accurately, verbalize drug info, keep appts for monitoring, use herbal supplements with caution and report use
general interventions related to drug therapy include... promoting health, preventing/decreasing need for drugs, using nondrug measures to enhance therapeutic effects or dec. adverse effects, teaching, individualizing care,, administering drugs and observing responses.
general techniques on evaluation of drug effectiveness directly observe pt. status, interview pt or others about pt response to drug therapy, checking appropriate medical records, including medication records and diagnostic test reports.
clinical pathways guidelines for care of pts with particular condition. include medical diagnosis, aspects of care related to medical diagnosis, desired pt outcomes and time frames for results.
Evidence based practice use research, keep up to date, consider levels of evidence (level a is strongest) interpret evidence in relation to one's expertise and pt choices, apply info to improve care, evaluate effectiveness of application.
nursing concern with herbal products and drug therapy not standardized, keep pt from seeing dr, interactions, not studied, don't tell dr. of use.
