Define Clinical Pharmacy. The area of pharmacy concerned with the science and practice of rational medication use.
Define Pharmaceutical Care. Includes the determination of the drug needs for a given individual.
What is The Hospital Formulary? A comprehensive list of every drug and therapeutic agent stocked by the pharmacy of that hospital.
What was the UCSF Ninth Floor Project? A plan that proposed the establishment and staffing of satellite pharmacies within the UC Hospital. This would eliminate the “error-prone, snail-paced existing system” of nurses acquiring drugs from the central pharmacy.
What is Collaborative Drug Therapy Management (CDTM)? Extended to allow pharmacists to function under protocol in licenses acute and intermediate care facilities to adjust dosages of a patient’s drug regimen.
Who was important in advancing Clinical Pharmacy? Forward-thinking hospitals and the VA were extremely important in progressing clinical pharmacy.
What is the difference between Morbidity and Mortality? Morbidity - refers to a diseased state, disability, or poor health due to any cause. Mortality - the condition of being mortal, or susceptible to death
What are the two key facts about clinical pharmacists? •Clinical Pharmacists provide care to their patients, not just clinical services. •A Clinical Pharmacist can work in any setting.
What is the difference between Medicaid and Medicare? •Medicaid – available for low income families. •Medicare – Age-based (over 65), or disability-based.
What is the future forecast for clinical pharmacy, according to the Joint Commission of Pharmacy Practitioners (JCCP)? “Pharmacists will be the health care professionals responsible for providing patient care that ensures optimal medication therapy outcomes.” (Includes responsibility for safe medication distribution and optimal medication outcomes)
(2) What is the future forecast for clinical pharmacy, according to the Joint Commission of Pharmacy Practitioners (JCCP)? JCCP’s vision for 2015-2020 – “Every patient who needs the clinical drug knowledge and skill of the pharmacist does in fact receive the services.
What is Medication Therapy Management (MTM)? Distinct service or group of services that optimize therapeutic outcomes for individual patients.
What are Patient-Centered Medical Homes (PCMH)? Establishment of community based health teams whose purpose is to support primary care practices. Goal is to have the best care integrated throughout all the offices.
What is Home-Based Primary Care (HBPC)? For patient’s independence at home; may include pharmacists.
What is "pay for performance"? Basically, the “more successful” the patient is (less hospital visits, less meds needed, etc.), the more that physician gets paid.
What are some barriers to Clinical Pharmacy? Residency Training Programs – Limited numbers of openings for the rising interest in clinical activities.
(2) What are some barriers to Clinical Pharmacy? Payment – Clinical pharmacists are currently NOT providers for Medicaid and Medicare for primary care services. Clinical Pharmacists are paid less than “normal” pharmacists.
