CCC - [Pharmacology] - Chapter 18
Question | Answer |
---|---|
calcium channel blocker, exerts a dilating effect on coronary and peripheral blood vessels by inhibiting the passage of calcium into vascular smooth muscle cells | Nifedipine |
Nifedipine effects | results in vasodilation and increased coronary blood flow, as well as lowered BP, increased cardiac output, and relaxed coronary artery spasms. Overall result is a reduction in cardiac workload and oxygen demand. |
Nitroglycerin protocol for acute angina attacks | 1 tablet SL, up to 3 tablets each 5 min apart. Call 911 after third tablet if pain has not subsided. |
Pain often resulting from ischemia or lack of oxygen in the myocardium. Ischemia creates an increased level of lactic acid and other metabolic products, which irritates the pain receptors that surround the heart. | Angina |
Angina Tx | typically treated by drugs that dilate the coronary blood vessels such as nitrates |
Common side effects of nitrates | hypotension, HA, flushing, and nausea |
only the sustained-release form of this drug should be used to treat hypertension | nifedipine |
Nitroglycerin transdermal application | generally worn for 12-14 hours period each day followed by a “patch off” period of 10-12 hours to prevent the development of tolerance. |
Nitroglycerin paste/ointment | should be applied over a 6x6 inch area of skin and covered by plastic wrap. Sites should be rotated to avoid dermal inflammation and sensitization |
Sublingual nitroglycerin | should be stored in their original container with a tightly closed cap in a cool place and should be discarded 6 months after opening original bottle. |
When should pt take nitroglycerin? | at the first sign of angina and should be sitting or lying down. A slight burning/tingling should occur as a positive sign of potency. |
Pt teaching for Coronary vasodilators | pts should be taught about specific drug therapy and learn how to moderate activities. Pts should identify and avoid situations frequently resulting in angina. Pts should also avoid tobacco because it has a vasoconstricting effect. Monitor for toxicity. |
Pt teaching for Peripheral vasodilators | Inform pt medications may cause flushing/warmth due to vasodilating effects and that this is a normal response. Educate about other side effects. Pts must not participate in activities that could injure the extremities and precautious when bathing feet. |
What vasoconstricting drug should be avoided by pt using vasodilators | tobacco |
