Antitussive-Prototype Dextromethorphan (Think, Orphans cough)
Dextromethorphan PO, Blocks cough reflex in medulla. Control of non-productive cough. Adverse: Respiratory depression.
Nasal Decongestant-Prototype Ephedrine
Ephedrine NS, Sympathomimetic. Used for common cold. Caution with pts with DM, Thyroid and Heart Disease and HTN. Adverse: Rebound Congestion
Oral Decongestant-Prototype Pseudoephedrine(Sudafed)
Pseudoephedrine (Sudafed) PO. Shrink mucous membranes by stimulating alpha-adrenergic receptors. Promote drainage, increase air flow. Caution with DM, Thyroid and Heart Diseases and HTN. Averse: Rebound congestion
Topical (Steroid) Decongestant-Prototype Flunisolide (Think Steroid-Solide)
Flunisolide NS. Block inflammatory response. May take days to weeks to provide relief, not used in emergencies. Do not use if infection is present. May become systemically absorbed if lacerations are in nares.
Antihistamine-Prototype Diphenhydramine (Benadryl)
Diphenhydramine PO,IM,IV. Dilates airways and decrease secretions. Can be used for tx of motion sickness. Allegra can be toxic with Ketoconazole or Erythromycin. Adverse: Cardiac Dysrhythmias.
Expectorants-Prototype Guaifenesin
Guaifenesin PO. Decrease adhesiveness and surface tension of fluids to allow easier movement. Relief of non-productive cough.
Mucolytics-Prototype Acetylcysteine
Acetylcysteine Inhale,PO. Break down of mucous to aid high risk respiratory pt in coughing up secretions. (Post Op pts, ect) Can also be used as tx for acetamenophen toxicity. Adverse: Bronchospasm, Stomatitis)
Xanthines-Prototype Aminophylline
Aminophylline PO,IV. Direct effect on smooth muscles in bronchi and blood vessels. Theophylline levels at 10-20mcg/ml. Adverse:(too much caffine)Life threatening arrhythmias, brain damage, respiratory arrest, death.
Sympathomimetic-Prototype Epinephrine
Epinephrine (Albuterol)SQ,IV,IM,Inhale. Beta-2 selective adrenergic agonist. Dilate bronchi. Adverse:Rebound congestion, cardiac arrythmias, bronchospasm.
Anticholinergic Bronchodialators-Prototype Ipratropium
Ipratropium Inhale. Block Vagus nerves effect on the respiratory tract. Do not give to pt with soy or peanut allergy. Adverse: Worsens Glaucoma, CNS, GI, Cough, Palpitations.
Steroids-Protptype Budesonide (Think steroid-sonide)
Budesonide Inhale. Decrease inflammation in airway. Not used in emergencies or with active infection. Adverse: Possible systemic absorption, local infections, rebound congestion.
Leukotrienes-Prototype Zafirlukast (Singulair)
Zafirlukast PO. Selectively blocks airway edema/mucous production. Not used in emergencies. D2D: Theophylline, Warfarin, ASA. Adverse: Generalized pain, infection, elevated liver enzymes.
Surfactant-Prototype Beractant
Beractant Intratracheal. Given to neonates at risk for or who have developed RDS. Suction before med and hold for 2 hours post med. Adverse: PDA, Pneumothorax, ect.
Mast Cell Stabilizers-Prototype Cromolyn
Cromolyn Inhale. Works at cellular level to inhibt Histamine. Not used in emergencies. D2D; Isoproterenol. Adverse: CNS, Dysuria, Nasal Congestion.
