How should Lantus be initiated in Type 1 diabetes? | 0.5-1U/kg/day (onset 1h) Use approximately 1/3 of the total daily insulin requirements. Short acting, premeal insulin should be used for the remainder of the requirements. |
How should Lantus be initiated in insulin naive Type 2 diabetes? | Starting dose is 10 units (or 0.2 U/kg) once daily, then increase by 1u/day until FPG <100mg/dL |
How should Lantus be converted if using once daily NPH insulin? | Recommended initial Lantus dose is the SAME as the dose of NPH that is being discontinued. |
How should Lantus be converted if using twice daily NPH insulin to once daily Lantus? | Use 80% of total NPH dose that is being discontinued. (This will lower the chance of hypoglycemia.) |
How is Lantus available? | 10ml vial (1000U/10ml) 3ml Cartridge system OptiClik (300U/3ml) 3ml SoloStar disposable insulin device |
What is Lantus? | LANTUS®, a basal insulin analog, is slowly released over 24 hours. It is given once daily at the same time each day. |
When should Apidra be given? | Administer within 15 minutes before a meal or within 20 minutes after starting a meal. Use in a regimen with an intermediate or long-acting insulin. |
How is APIDRA 100 units/mL (U-100) available? | • 10 mL vials • 3 mL cartridge system for use in OptiClik ® (Insulin Delivery Device) • 3 mL SoloStar® prefilled pen |
How is Apidra dosed? | Add prandial insulin in a stepwise fashion, starting with 1 dose at the largest meal. Start with 4U, then every 3D, increase by 2U until PPBG in range (<180mg/dL 2h post meal. |
