Altered Nutrition: less than body requirements | Individual is experiencing an intake of nutrients insufficient to meet metabolic needs |
Altertered nutrition: More than body requirements | Individual is experiencing an intake of nutrients that exceeds metabolic needs |
Altered Urinary elimination | Individual experiences a disturbance in urine elimination |
Constipation | Decrease in persons normal frequency of defecation accompanied by difficult or incomplete passage of stool and passage of excessively hard, dry stool |
Diarrhea | Passage of loose, unformed stools |
Fluid volume excess | Individual experiences increased isotonic fluid retention |
Fluid volume deficit | Individual experiences decreased intravascular, interstitial and or intracellular fluid. This refers to dehydration, water loss alone without changes in sodium |
Fatigue | An overwhelming sustained sense of exhaustion and decreased capacity for physical and mental work at usual level |
Ineffective airway clearance | Inability to clear secretions or obstructions from the respiratory tract to maintain a clear airway |
Impaired swallowing | Abnormal functioning of the swallowing mechanism associated with deficits in oral, pharyngeal or esophageal structures of function |
Impaired Skin integrity | Individual has altered epidermis |
Impaired physical mobility | Limitation in independent, purposeful physical movement of the body or of one or more extremities |
Knowledge deficit | Absence of defiency of cognitive information related to a specific topic |
Pain | An unpleasant sensory and emotional experience arising from actual or potential tissue damage, sudden or slow onset of any intensity from mild to severe with anticipated or predictable end and duration less than 6 months |
Risk for injury | A state in which an individual is at risk for injury as a result of environmental conditions interacting with the individual's adaptive and defensive resources |
Risk for infection | The state in which individual is at increased risk for being invaded by pathogenic organisms |
Self Care deficit | Impaired ability to perform or complete ADLs for oneself |
