patient plan of care it is written by the registered nurse. it serves as a course of action to assist the patient to achieve optimum wellness. it is a way for the nursing team to communicate. they provide a structure for assessing, planning, implementing, and evaluating
patient plan of care 2 individualized care. one reason for the written care plan is to ensure that continuous and consistant care is provided for each patient.
the plan of care provides the following specific instructions regarding care to be given. information needed prior to giving care. guidelines for continuity of care. information essential for organizing and planning work or special duties.
weighing and measuring weights and heights are recorded to provide an important database. accuracy is important because some medictions are ordered based on weight.
discharging the patient written permission from a doctor is required for the patient to be discharged. in many institutions, the patient must be taken to the business office, cashier, or discharge desk before the patient leaves the facility.
