wheal superficial, raised, transient, and erythematous
urticaria (hives); wheals coalesce to form extensive reaction
cyst encapsulated fluid-filled cavity in dermis or subcutaneous layer, tensely elevating skin
vesicle elevated cavity containing free fluid; up to 1 cm; (blister)
bulla larger than 1 cm diameter; usually single chambered; superficial in epidermis; thin-walled (ruptures easily)
pustule turbid fluid (pus) in cavity; circumscribed and elevated
crust thickened, dried-out exudate left when vesicles/pustules burst or dry out; color varies
scale compact, desiccated flakes of skin
fissure linear crack with abrupt edges; extends into dermis
erosion scooped out but shallow depression; superficial; moist but no bleeding
ulcer depression extending into dermis; irregular shape; may bleed
scar normal tissue is lost and replaced with connective tissue
lichenification prolonged intense scratching eventually thickens the skin and produces tightly packed sets of papules
excoriation self-inflicted abrasion; superficial; scratches from intense itching
atrophic scar resulting skin level depression with loss of tissue; thinning of epidermis
keloid hypertrophic scar; skin level is elevated by scar tissue
pattern injury wound whose shape suggests the instrument that caused it
hematoma bruise that elevates skin and seen as swelling
contusion (bruise); large patch of capillary bleeding into tissues
