Internal Medicine is the medical specialty dealing with the prevention, diagnosis, and treatment of adult diseases. Physicians specializing in internal medicine are called internists.
Medical Condition: Heart Disease Heart disease is a broad term used to describe a range of diseases that affect the heart.
Medical Condition: Diabetes Diabetes is a metabolic disease in which the body’s inability to produce any or enough insulin causes elevated levels of glucose in the blood.
Medical Condition: Anemia Anemia is a condition in which the body does not have enough healthy red blood cells. Red blood cells provide oxygen to body tissues
Medical Condition: Debility/Functional Decline Debility is physical weakness caused by illness or old age. Functional decline is the decrement in physical and/or cognitive functioning and occurs when a person is unable to engage in activities of daily living, as is encountered during hospitalisation.
Medical Condition: Stomach Disorders Stomach Disorders are the diseases or disorders that affect the stomach.
Medical Condition: Constipation Constipation a condition in which there is difficulty in emptying the bowels, usually associated with hardened feces. You may have pain while passing stools or may be unable to have a bowel movement after strain or pushing for more than 10 minutes.
Medical Condition: Infections Infection is an invasion of the body by pathogenic microorganisms.
Medical Condition: Depression Depression a serious medical condition in which a person feels very sad, hopeless, and unimportant and often is unable to live in a normal way.
Medical Condition: Cardiovascular Conditions Cardiovascular Conditions are disorders or diseases of the heart and blood vessels.
Medical Condition: Asthma Asthma is a disorder that causes the airways of the lungs to swell and narrow, leading to wheezing, shortness of breath, chest tightness, and coughing.
Medical Condition: Emphysema Emphysema is a condition in which the air sacs of the lungs are damaged and enlarged, making you progressively more short of breath.
Medical Condition: Deep Vein Thrombosis Deep venous thrombosis is a blood clot that forms in a vein deep inside a part of the body. It mainly affects the large veins in the lower leg and thigh.
Medical Condition: Hypertension hypertension (High blood pressure) is a chronic medical condition in which the blood pressure in the arteries is elevated. Is when your blood pressure is 140/90 mmHg or above most of the time. Abnormally high blood pressure.
Medical Condition: Liver Disease liver disease applies to many diseases and disorders that cause the liver to function improperly or stop functioning. Abdominal pain, yellowing of the skin or eyes (jaundice), or abnormal results of liver function tests suggest you have liver disease.
Medical Condition: Urinary Tract Infection A urinary tract infection (UTI) is an infection in any part of your urinary system — your kidneys, ureters, bladder and urethra. Most infections involve the lower urinary tract — the bladder and the urethra.
Medical Condition: GallBladder Gallbladder is a pear-shaped organ under your liver. It stores bile, a fluid made by your liver to digest fat. As your stomach and intestines digest food, your gallbladder releases bile through a tube called the common bile duct.

Services: Medication Management Medication management is defined as patient-centered care to optimize safe, effective and appropriate drug therapy. Care is provided through collaboration with patients and their health care teams.
Services: Pain Management Pain Mgmt is a medical approach that focus on easing the suffering and improving the quality of life of those living with pain
Services: IV Infusion the process of administering a solution intravenously.
Services: Skilled Assessment Skilled Assessment is performed by our nurse to evaluate the residence, the patients, level of functions at home.
Services: Injections/Teaching Injections/Teaching: our experienced nurses will review how medications are administered and teach the patient and/or caregiver during a one on one injection teaching class.
Services: Medical Social Worker assess the psychosocial functioning of patients and families and intervene as necessary. Connecting patients and families to necessary resources in the community; provide psychotherapy, supportive counseling or grief counseling.
Services: Physical Therapy Physical Therapy treats or manage a physical disability, malfunction, or pain by by physical methods like exercise, massage, hydrotherapy, etc.It helps improve or restore physical function and fitness level to make daily tasks and activities easier.
Services: Occupational Therapy OT helps patients recover or develop skills needed for the activities of daily living, including self-care and independent living. This includes patients who have had strokes or recovering from certain surgeries, including hip replacements.
Services: Speech Therapy Speech Therapy is the treatment to help restore or improve the power of speech, through physical exercises to strengthen the muscles used in speech (oral-motor work) to adults who have lost it or partly lost it through accident or illness.
Services: Educating Patient/Family is the process to educate and provide information and techniques to patients and their caregivers (family) that will alter their health behaviors and improve their health status.
Services: Bowel/Bladder Training The goal of bowel and bladder training is to help patients learn various techniques and exercises, as well as adopt simple lifestyle changes to help control the incidence of incontinence.
Services: Wound Care Education Our homecare team works closely with our patient, family, and caregivers to provide education on how to assist with the wound healing process.
Services: Catheter Insertion Care catheterisation is the process of inserting a catheter (hollow tube) into the bladder for the purpose of draining urine or intravenous for drug therapy. Our nurses will monitor the catheter to prevent any complications and minimize any risk of infection.
Services: Fluid/Electrolyte Monitoring Nurses will assess, monitor and manage fluid and electrolyte needs in surgical patients.
Services: Psychiatric RN treat patients diagnosed w mental illness like schizophrenia, bipolar disorder and depression. They’re also trained in behavioral therapy, which allows them to teach patients and their family, how to deal w challenges that go along w psychiatric disorders
