N112 - NSAIDs for test 1

Platelet aggregation | ASA (aspirin) inhibits the COX enzyme in the platelet irreversibly such that the platelet cannon regenerate this enzyme its effect last 7 days |
COX 1 enzyme | role is to maintain intact GI mucosa; GI (gastrointestinal) side effects are #1 with first generation NSAIDs, also peptic/duodenal ulcers, GI bleeding, nausea, abdominal pain, and indigestion/heartburn |
COX 2 enzyme | side effects are fatigue, dizziness, lower extremity edema & HTN (hypertension) |
Salicylate toxicity or aspirin toxicity | symptoms include tinnitus and/or hearing loss |
Nursing consideration 1 | teach clients to NOT take alcohol & ASA (aspirin) together with NSAIDs |
Nursing consideration 2 | teach clients to take with full glass of H2O & remain upright for 30 minutes following administration |
Nursing consideration 3 | advise clients to look for changes in stool (melena or dark/black stools) |
Evaluate effectiveness of the drug | increased joint mobility, decreased discomfort/pain, fever reduction, reduction in stroke/MI (myocardial infarction) |
Drug to drug interactions | Alcohol, ASA & anticoagulants, increase GI symptoms, increased bleeding |
