QuestionAnswer
Questions to ask: Have you had any cough, sob, chest pain? do you smoke? any medications? allergies? any concerns today?
Appearance how does the pt appear today? how are they sitting? any acute distress noted?
Check AP lateral ratio and costal angle. what is normal?
Note skin color what is normal?
Note tracheal position is it midline?
Palpate for crepitus and tenderness; tactile fremitus; symmetrical posterior chest expansion?
Auscultate Where are bronchial, bronchovesicular and vesicular breath sounds heard? how many lobes on right? on left?
Listen: assess breath sounds, are they clear throughout? what would crackles sound like? rhionchi? wheezes?
If concerned about pneumonia, what tests could you do to test for consolidation? Bronchophony, Whispered Pectoriloquy, Egophony
