What are the different categories of antidepressants? | Tricyclic - TCAs, Selective serotonin reuptake inhibitors - SSRIs, Monoamine oxidase inhibitors - MAOIs, Heterocyclic/Tetracyclics - 2nd Generation antidepressants. |
What are they used for? | Affective disorder - depression, smoking cessation - Wellbutrin. |
What are the actions of antidepressants? | Antidepressants cause adaptive changes in the serotonin & norepinephrine receptor systems and help to restore these levels within the nerves. |
What are TCA side effects? | Drowsiness/sedation, dry mouth, N/V, arrhythmias, impotence, constipation, altered libido. |
What are SSRI & 2nd generation side effects? | Insomnia, weight gain, sexual dysfunction, GI symptoms, Serotonin syndrome - delirium, agitation, tachycardia, sweating, myoclonus - involuntary twitching & shivering. |
What are MAOI side effects? | Orthostatic hypotension, hypertensive crisis, anorexia, dry mouth & impotence. |
What are some patient teachings for antidepressants? | Take with meals to avoid GI upset, drug may take 4-6 weeks to obtain max response, do not stop taking abruptly, avoid alcohol. Pts taking MAOIs should avoid tyramine - in aged cheese, smoked meats, red wines - potential for hypertensive crisis. |
