N113 - Meds for Exam 1

QuestionAnswer
Fentanyl uses Used for pain management. Used in epidural or intrathecal analgesia, alone or in combination with a local anesthetic.
Fentanyl side effects Common side effects include: headache, pruritus, nausea, vomiting, diaphoresis, difficulty breathing, confusion, dizziness, diarrhea, constipation, decreased appetite.
Fentanyl interactions Benzodiazepines may increase risk for hypotension, respiratory depression. Buprenophine may decrease effect. CNS depressants may increase respiratory depression, hypotension.
Fentanyl nursing considerations Obtain baseline vitals. Assess character of pain. Assist with ambulation, monitor vitals. Assess for respiratory depressions, Narcan should be available as antidote. If given as epidural, assess BP for hypotension & effects on FHR.
Colace uses It is used to avoid constipation and straining during elimination.
Colace side effects Occasional mild abdominal cramping or throat irritation.
Colace interactions May increase the absorption of mineral oil and danthron.
Colace nursing considerations Encourage fluids, assess bowel sounds for peristalsis. Monitor daily bowel activity. Teach pt to use fiber, fluids & activity to decrease constipation.
Fentanyl trade/brand name Sublimaze
Fentanyl dosage IM: 50-100mcg; IV: 25-50 mcg. Epidural- 1-2 mcg with 0.125% bupivacaine at rate of 8-10 ml/hr
Colace generic name Doscusate
Methergine generic name Methylergonovine
Methergine dosage PO 0.2 mg 3-4 times/day cont for 7 days. IM/IV Initially, 0.2 mg. May repeat q2-4h for no more than 5 doses.
Methergine uses To manage excessive postpartum bleeding.
Methergine side effects Hypertension, nausea, uterine cramping, vomiting, abdominal, pain, diarrhea, dizziness, diaphoresis, tinnitus, bradycardia, and chest pain.
Methergine interactions Vasoconstrictors, vasopressors may increase effect.
Methergine nursing considerations Obtain baseline B/P, pulse, and serum Ca levels. Asses bleeding prior to adm. Monitor uterine tonicity, bleeding, B/P, pulse (q15 min until stable approx 1-2hrs.). Assess extremities for color, warmth, movement, pain. Report chest pain immediately.
Nubain generic name Nalbuphine hydrochloride
Nubain dosage IM/IV 10 mg q3-6h prn, not to exceed 20mg/dose or 160mg/day.
Nubain uses Labor & post c-section pain
Nubain side effects Sedation, confusion, sweating, transient sinusoidal-like FHR rhythm
Nubain interactions Alcohol, CNS depressants may increase CNS or respiratory depression. Hypotension. MAOIs may produce severe reaction. Buprenorphine may decrease effects of nubain.
Nubain nursing considerations bed rails up, obtain vital signs, if resp are ≤ 12/min withhold and call physician. Assess pain. Following adm.: Monitor resp., B/P, rate/quality of pulse Assess for relief of pain. May precipitate withdrawal symptoms in mother and newborn
Prenatal vitamin brand name Zenate
Prenatal vitamin nursing considerations Take med as prescribed. Nausea, mild aftertaste, constipation, and black stool are normal. Take each dose with full glass of H2O. Measure doses correctly. A missed dose can be taken as soon as remembered, but never double a dose.
Tylenol #3 uses Relief of post-partum pain
Tylenol #3 dosage Each tablet contains 325 mg acetaminophen and 15 mg codeine. Normal dose is 1-2 tablets q3-4 hours
Tylenol #3 interactions Increased bleeding with warfarin.
Tylenol #3 nursing considerations Assess pain level prior to adm. and again following. Obtain vital signs, if respirations are <12/min.; withhold med and contact physician.
