Hypothyroidism - Pharm for Nursing

Mechansim of Action Induce changes in Metabolic rate,increase O2 consumption, Body Temp, BVolume, Overall cellular growth and differentiation, stimulate CV system by ↑ sthe sensitivity of heart to catecholamines and ultimately ↑ CO.
Indications Used to diagnose hyperthyroidism, prevention / treatment of various goiters, replacement Hormone therapy, Pt. who have had thyroid removed/destroyed.
Contraindications Allergy, Recent MI, Adrenal Insufficiency, hyperthroidism.
Adverse Effects - CV TachyC, palpitation, angina, dysrhymia w/ life fatal irreg., hptertension, cardiac arrest
Adverse Effects - CN Insomnia, tremors, headache, anxiety
Adverse Effects - GI Nausea, diarrhea, I/D in appetite, cramps
Adverse Effects - other Menstrual Irreg., Decrease weight, sweating, heat intolerance, fever.
Interactions with Anticoagulants inhance anticoagulant effect.
Interactions with Digitalis Glycoside Decrease Digitalis levels
Interactions with Cholestyramine binds to thyroid hormone in GI, decrease absorption of both drugs
Interactions with Diabetics May require Pt. dose to ↑.
Interactions with Epinephring Pt. with coronary disease, may induce coronary insufficencies.
Levonthyrovine (Synthroid, Levoxyl) Synthetic Levonthyrovine (thyroid hormone T4) Orals for Hypothyroidism, IV for Myxedema
Levonthyrovine 1/2 Life 9-10 days, Peak 3-4 weeks. risk for toxicity
Toxicity of Levonthyrovine Manifest by Decrease Weight, TachyC, nervousness, tremors, headache, insomnia, menstral irreg., Cardiac irreg. 99% protien bound so it stays in body longer increasing risk of toxicity.
Liothyronine (cytomel, Triostat) Synthetic Liothyronine (thyroid hormone T3)
Liotrix (Thyrolar) Synthetic Thyroid hormone T3-T4 combination
Thyroid (Armour, Thyroid, S-P-T) Dessicated animal thyroid gland.
