Safety Measures for Using Restraints

Use correct size and restraint per care plan | Never borrow another resident's device. |
Apply a restraint only after receiving instructions, demonstration of proper application technique | Never guess how a device is to be used or use a device without proper knowledge. |
Use only approved devices. | Never use sheets, towels, ropes, bandages, or other items to restrain a resident. |
Use intact restraints. | Never use restraints with tears, frayed fabric, missing hooks, loops, straps or other damage. |
Position person in good alignment. | Never position a resident after the restraint is applied. |
Restraint should be snug but allow movement of the body part. | Never restrict a resident's breathing. |
Flat side of hand slides easily under body restraing or two fingers slide under a wrist or mitt restraint. | Never tie a device so tight as to cause physical harm or too loose to allow for entrapment. |
Bony area is padded. Restraint is applied over clothing. | Never restrain a resident to position him/her on the toilet. |
Secure the straps out of the person's reach. | Never permit other residents to play with or release a knot. |

Check the person's circulation above and below the restraints every 15 minutes. Fingers or toes should be warm and pink. | Never continue using a restraint if you cannot feel a pulse, the skin is cold, pale or blue in color, person complains of numbness, tingling, or pain or the skin is damaged. |
Remove the restraint and reposition the person every 2 hours. Meet basic needs. | Never leave a nutritional or elimination need unattended; especially if th resident is frequently incontinent. |
Keep the signal light within reach at all times. | Never place person in an unsupervised area. |
Keep scissors in your pocket to cut the device in the event of an emergency. | Never be unable or unprepared to provide first aid and emergency care at any time. |
Report and record the use and effects of the device. | Failure to ensure that the person's quality of life is protected is abuse. |
