Bipolar disorders

Meds: Mood Stabilizer / antiepileptic drugs

mood stabilizing meds / carbamazepine, valproic acid, lamotrigine treatment & prevention of relapse of mania & depressive episodes.
Cabamazepine complications (1) CNS effects: nystagmus, double vision, vertigo, staggering gait, headache (subside in a few weeks). RN cons: low doses initially, then increase dosage. avoid driving & other activities that require alertness @ beginning of treatment. adm. @ bedtime.
carbamazepine complications (2) blood dyscrasias, teratogenesis, hypo-osmolality, skin disorders; report Stevens-Johnson syndrome, hepatotoxicity don't give to children less than 2 years old,
carbamazepine interactions concurrent w/oral contraceptives, warfarin decrease the effects of these meds. use alternate form of birth contl. grapefruit juice increases carbamazepine levels. avoid grapefruit juice. phenytoin & phenobarbital decrease the effects of carbamazepine.
carbamazepine contraindications/precautions bonemarrow suppression or bleeding disorders
Lamotrigine complications double or blurred vision, dizziness, headache, n/v, skin rashes include stevens-johnson syndrome - withhold med, & notify HCP
valproic acid complications GI effects, hepatotoxicity, pancreatitis d/c med, thrombocytopenia - notify HCP teratogenesis, weight gain
