S.O.A.P. Method A way to organize information about patients. Acronym for Subjective, Objective, Assessment, Plan.
Diagnosis What a health care provider thinks a patient has, based on symptoms exhibited and results of an exam Abbr: Dx
Differential diagnosis A list of conditions a patient may have, based on symptoms exhibited and results of an exam. Used when Dx is unclear or not obvious Abbr: DDx
Acute Symptom onset is recent, sudden, sharp, or severe
Chronic Symptom onset has been long-building or ongoing
Auscultation To listen. Usually is referring to lung, heart, and organ sounds heard with a stethoscope
Percussion Tapping on something to listen to the sound it makes. Used to determine size and borders of organs, and determine presence or absence of fluids
Palpation To feel. Using fingers and hands to feel during examination
Etiology The cause of a disease or condition
Remission To get better or improve. Most often used when discussing cancer. Remission DOES NOT mean cured!
