What do you evaluate with Abdominal Doppler? Abdominal AO, Mesenteric arts (celiac axis, Hepatic A, Splentic A, SMA), Renal As & Vs, Portal venous system (Portal Vs, SMV, & Splenic V), IVC, and Hepatic Vs
Reasons for eval and Doppler interrogation for Abdominal Doppler? Aneurysm, Arterial stenosis, Ischemia, HTN, Thrombsis, Budd-Chiari syndrome, TIPS eval, Graft eval, Transplant eval
What TD do you need for abdominal scanning? 2-5 MHz TD curved
What do you not do to the spectral waveform or color flow in abdominal doppler studies? DO NOT INVERT spectral waveforms or color flow
Where does the Abdominal AO begin? Begins at the level of the 12th thoracic vertebra (passes through the aortic hiatus)
Celiac Artery facts: AKA Celiac trunk, Celiac axis, First major anterior branch of aorta, Gives rise to Hepatic, Splenic and Left Gastric Arteries
Superior Mesenteric Artery facts: Next anterior branch of the aorta, Arises just below the celiac artery, Supplies most of the small intestine and some large intestine
Inferior Mesenteric Artery Facts: Last anterior branch of Aorta, Arises 3-4 cm above aortic bifurcation, Mainly supplies the large intestines
Renal Artery facts: Lateral branches of Ao, Arise just below the SMA, Right Renal A is longer & slightly thinner than Left, Right Renal A courses posterior to the IVC, Both Renal arts approach kidney posterior to renal veins
What are the Renal Vasculature Arteries? Main renal artery (@ hilum), Segmental renal artery (renal sinus), Interlobar arteries, Arcurate arteries (renal cortex), interlobular arteries
What are the vessels of the Abdomen- arteries? Testicular/Ovarian As (anterolateral branches of the Ao, Arise just below the renal As), Lumbar As (arise posteriorly off the Ao), Middle sacral artery (single A arising off the posterior Ao, Arise just above the aortic bifurcation)
IVC facts: Courses through abdomen to the right of the Ao
What are the IVC major tributaries? Renal Vs (R&L), Hepatic Vs (R,M, &L), Lumbar Vs, Ovarian/Testicular Vs
Where does the left Ovarian/Testicular V typically drain? Into left renal vein
Hepatic A carries what? Oxygenated blood into the liver, Carries 30% of total blood flow into the liver
Portal V carries what? Remaining 70% of blood, Formed at junction of splenic& SMVs, Begins posterior to the neck of the pancreas and anterior to IVC
Hepatic Vs (right, middle & left) does what? Drain from liver into IVC
