This regulation outlines the performance review and in-service education components that are required for nurse aides. Nurse aides must receive a minimum of 12 hours of in-service education per year. Ftag 730- Record reviews will show surveyors if a performance review of each nurse aide was conducted at least once every 12 months. Annual in-service education hours calculation should be based on employment date rather than the calendar year.
“A facility must maintain all resident assessments completed within the previous 15 months in the resident’s active record.” Ftag 639- Facilities are specifically directed to use the results of those 15 months of assessments to “develop, review and revise the resident’s comprehensive care plan.”
“A facility must be administered in a manner that enables it to use its resources effectively and efficiently to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident” Ftag 835- Surveyors must conduct an investigation that demonstrates how the Administration knew/should have known that there was a deficient practice, as well as how lack of Administration contributed to the issue.
Residents have the right to be fully informed of their total health status, including medical condition, in a language that they can understand. Ftag 552- This means that communication is made in plain language where any medical/technical jargon is explained in a way that non-health professionals can understand.
residents must receive, and the facility must provide, at least three meals per day. Ftag 809
Requires nursing facilities to ensure that residents who have prostheses are provided with care and assistance to wear and use the prosthetic device. Ftag 696- Care/ assistance given must be in accordance with the person-centered care plan. Requirement means that the non-availability of program funding does not relieve the facility of its obligation to ensure residents received all services needed.
Residents have the right to be informed in advance of the types of care to be provided and the type of caregiver that will be providing that care. Ftag 552- This includes having a physician or other health professional respond to resident/representative concerns as well as when a change in treatment is being proposed.
“Suitable, nourishing alternative meals & snacks,” – meals/snacks that are of similar nutritive value as meal/snack that is offered during normally scheduled meals. Ftag 809- Must be provided to residents who prefer to eat at non-traditional times or outside of scheduled meal times, so long as this is consistent with the care plan.
Allows self-administration of meds if Interdisciplinary Team (IDT) has determined it is clinically appropriate & safe to do so. Ftag 554- Facilities must show via documentation how IDT decided the resident is capable. Must have process that validates resident has taken self-administered medication. These med errors are not counted in the facility’s med error rate.
Residents have the right to be informed in advance of the risks/benefits of proposed care and treatment options, as well as alternatives. Ftag552- There is a major responsibility to keep residents/reps informed of health status, care provided, & giving resident option to choose treatments in advance. Guidance addresses use of qualified sign language & language interpreters for residents.
Meals must be served “at regular times” based on normal mealtimes in the community, or, adding in the person-centered focus, may also be served in accordance with resident needs, preferences, requests & plan of care. Ftag 809
Responsible for supervising care & participating in resident assessment/care planning, monitoring & addressing change in medical status, writing medication & treatment orders, as well as responding when contacted by facility. Ftag 710- Physician is responsible for conducting routine visits, but if he/she delegates follow-up visits to an non-physician practitioner the physician’s supervision responsibilities remain.
Requires facilities maintain all mechanical, electrical and patient care equipment in safe operating condition. Ftag 908- “All” types of equipment includes everything from unit refrigerators, kitchen equipment to laundry machines. The regulation also states that all equipment must be maintained according to manufacturer’s recommendations.
Ability to obtain records, both medical & personal. Access to records, or copies of these records, must be provided, even if the request is only made orally. Ftag 573- Need to be provided in a form/format requested by resident, a “readable” hard copy or other format may be provided if agreed upon by the facility & individual requesting records. Access to be provided w/in 24 hrs except on weekends & holidays.

Discarding expired meds, labeling/dating/discarding multi-use vials, cleanliness of med rooms & carts, med refrigerator temps are all regulated under this Ftag? Ftag 761- Labeling & Storage of Drugs & Biologicals.
Ensure all residents receive the proper treatment & assistive devices required to maintain their hearing and vision. Ftag 685- Requires the facility assist residents w/ making appointments & arrange for transport to/from doctor’s office specializing in vision or hearing impairments. They are tasked w/ assisting resident/rep w/ locating & using available resources.
Ensure ordered x-rays/ diagnostic tests are completed, notification of medical practitioner of results (especially abnormal) is done & appropriate medical follow-up conducted should be a basic standard of practice Ftag 777- Dr/Practitioner notification of test results that are “outside of reference ranges” or as directed by practitioner are clearly spelled out. P & P should be developed in consultation w/ Medical Director & follow current standards of practice.
This Ftag outlines a nursing facility's requirements for not admitting new residents with a mental disorder or intellectual disability except where certain criteria have been met. Ftag 645
What Ftag does pre-employment screening requirements fall under? Ftag 607: Develop/Implement policies & procedures to prevent Abuse/ Neglect/ Mistreatment.
What Ftag outlines the facility’s responsibilities towards screening of a potential employee prior to employment? Ftag 606: This tag also states facility's responsibility to make a report to State nurse aide registry/ licensing authorities when it becomes aware of court actions against an employee that would make them unfit for hire in a nursing facility.
This Ftag requires that facilities need to identify required services needed by a resident & ensure that these services are provided. Ftag 745: Note- all services do not necessarily have to be provided by a qualified social worker except as required by State law.
This regulation regulation expects ongoing communication & coordination between departments & staff to ensure residents’ nutritional & dietary needs are met according to resident’s assessment & care plan, & that plan of care must include choices. Ftag 800: Provided Diet Meets Needs of Each Resident. There is a clear emphases on resident choice. Common issues cited; those who dine in the room no being provided with info about alternate options, identified food preferences not being addressed.
Visitation rights must not infringe on the rights of other residents, this means the facility is responsible for finding a location for visits other than the resident’s room if needed, such as when resident’s roommate is asleep. Ftag 563: Right to Receive/Deny Visitors. Facilities must have written P&Ps regarding visitation rights for residents.
True or False: Immediate family means only blood relatives? False: Interpretive Guidance (IG) not restricted to blood relatives, & instead refers to multiple family types that provide “a supportive, caring unit,” which is defined by resident.
True or False: Immediate family are not subject to any visitation hour limitations. True: Unless limitations have been set by the resident.
Staff orientation and training must cover all forms of abuse, neglect, misappropriation of resident property, exploitation and dementia management; Which Ftag is this? Ftag 943: Outlines requirements related to staff training on abuse, neglect, exploitation & misappropriation of resident property, & defines specific topics that need to be covered as part of this required education.
Requires that a facility have a policy in place that identifies circumstances when the loss/damage of dentures is the facility's responsibility & when a resident may not be charged for a replacement. Both Ftag 790 & 791: F790 is for SNFs and F791 is for NFs
This regulation requires that a negative or potentially negative outcome for a resident is determined to be related to the facility’s failure to meet these standards & surveyors determine that a deficient practice has occurred... Ftag 658: Facility should be cited under both F658 as well as the appropriate area where concern was identified.
What are some areas this tag can be cited? Failure to provide basic life support, failure to follow professional standards of nursing practice for unresponsive resident with a hypoglycemic episode, failure to meet professional standards for med administration of insulin.
