Mechanism of Action for NSAIDS? inhibit prostaglandin synthesis; prostaglandin cause the uterus to contract
Girls that have amenorrhea are at risk for what? osteoporosis because of prolonged hypoestrogenic states
Increase risk of cervical cancer when exposed to? HPV
ACOG: When should girls get first cervical cytology screening? 3 years after first intercourse or by age 21
ACOG: Women up to age 30 should have cervical screening cytology how often? Annual because younger women are at higher risk
ACOG: "LOW RISK" Women over age 30 should have cervical screening cytology how often? A "LOW RISK" pt after 3 negative screenings on 3 consecutive visits then can prolong screen to every 2-3 yrs
ACOG: "HIGH RISK" Women over age 30 should have cervical screening cytology how often? Annual because "HIGH RISK" includes multiple partners, HIV etc.
Assessment Finding: Thick, white, curd-like vaginal discharge usually with c/o intense itching Vulvovaginal Candidiasis; yeast infection
If suspect candidiasis, why do KOH prep? Looking for budding yeast thus confirmation of Dx
Tx for Typical Yeast? Fluconazole (Diflucan) PO; OTC antifungals work pretty good too so if pt taking OTC and not better give something to tx Atypical yeast
Tx for Atypical Yeast? Terconazole (Terazol) suppository or cream
What tx for pregnant women with candidiasis? NO PO; use topical "azole" for 1 week; no 3 day tx
Another name for Stein Leventhal Syndrome is? Polycystic Ovarian Disease (PCOS)
Assessment Findings: Oligomenorrhea or amenorrhea; hyperandrogenism, hyperinsulinemia, possibly obesity PCOS
What is Oligomenorrhea? fewer than 9 menses per yr
What is Amenorrhea? no menses for 3 or more months
Describe Hyperandrogenism: hirsutism (facial hair), acne, seborrhea, alopecia, voice changes, hypertrophy of the clitoris
Tx for PCOS if pregnancy is desired? (maybe not so comfortable in giving) Clomiphene (Clomid) causes "litter"
Tx for PCOS if pregnancy is disired? (comfortable in giving) Metformin
Black kohash (phytoestrogens) an herb is contraindicated in which women? Hx of breast Ca
Combined OCs are contraindicated in a pt with what? DVT
A 15 year old female has never menstruated what is important to assess? Tanner Stage
At what age no matter the Tanner stage should you be concerned that the girl has not started menstruating? Age 16
When should we advise backup method of birth control even if on combined OCs? If the pt is on Antibiotic
The whiff test is used to test for? Bacterial Vaginosis (BV)

Clue cells indicate what? bacterial vaginosis (BV)
Hyphae and spores indicate what? candida, yeast
What is the treatment for BV? metrondiazole (Flagyl)
What education should we tell pts taking metrondiazole? Avoid alcohol; produces disulfiram effect where there is increase in acetaldehyde causing "hangover" effect
