Severe Migraines | Triptan preps, Ergotamine derivatives, & isometheptene combos. SE HA, medication overuse HA |
Triptan | serotonin receptors, Zolmitriptan (ZOMIG), electriptan (REPLAX) Sumatriptan (IMITREX) contraindicated in ischemia heart disease, cerebrovasc ischemia, HTN, PVD, |
Patient Instructions for Triptan | Take as soon as migraine symptoms develop, report chest pain or tightness immediatly, use contraception while on this medication- not safe for pregnancy, common SE include: flushing, tingling, and a hot sensation. Do NOT take w/SSRI's st. john's wort |
Ergotamine | Cafergot- taken at start of HA,up to 6tabs in 24hrs,Dihydroergotamine (DHE) IV, IM or Nasal Spray- Migranal w/antiemetic not to be given w/in 24hrs of triptan |
Midrin | combo drug containing APAP, isometheptene & dichloralphenazone excellent option for when ergotamines do not work |
Opioids & Barbituates | Should be avoided if at all possible because they are addictive some actually cause migraines Feorocet |
