Severe Migraines Triptan preps, Ergotamine derivatives, & isometheptene combos. SE HA, medication overuse HA
Triptan serotonin receptors, Zolmitriptan (ZOMIG), electriptan (REPLAX) Sumatriptan (IMITREX) contraindicated in ischemia heart disease, cerebrovasc ischemia, HTN, PVD,
Patient Instructions for Triptan Take as soon as migraine symptoms develop, report chest pain or tightness immediatly, use contraception while on this medication- not safe for pregnancy, common SE include: flushing, tingling, and a hot sensation. Do NOT take w/SSRI's st. john's wort
Ergotamine Cafergot- taken at start of HA,up to 6tabs in 24hrs,Dihydroergotamine (DHE) IV, IM or Nasal Spray- Migranal w/antiemetic not to be given w/in 24hrs of triptan
Midrin combo drug containing APAP, isometheptene & dichloralphenazone excellent option for when ergotamines do not work
Opioids & Barbituates Should be avoided if at all possible because they are addictive some actually cause migraines Feorocet
