Dopamine Agonists | mimic dopamine, most effective in 1st 3-5yrs, include apomorphine (Apokyn), pramipexole (Mirapex), ropinirole (Requip) & rotigotine (Neupro) transdermal patch, to maintain consistent level of dopamine |
Dopamine Agonist SE | Orthostatic Hypotension, Hallucinations, sleppiness & drowsiness, avoid operating heavy machinery & driving, not for elderly |
Sinemet | combo Levodopa-carbidopa almost all pts are on this at some time, take before meals, LT use leads to dyskinesia |
COMT's | inactivate dopamine & prolong action of levodopa. entacapone (Comtan) used in combo w/levodopa, Stalevo is a combo of levodopa, carbidopa, and entacapone |
MAO-B Inhibitors | Entacapone (Comtan) & Selegiline (Deprenyl, Eldepryl) given w/levodopa early mild disease. New drug rasagiline mesylate (Azilect) |
MAO-B Inhibitor SE | Avoid foods, beverages, and drugs that contain tyramine including aged smoked or cured foods & sausage. Avoid red wine & beer |
Dopamine Recepter Antagonist | bromocriptine mesylate (Parlodel) used when other drugs are no longer effective |
Antiviral Drug | Amantadine (Symmetrel) give early in disease tx and with Sinemet |
Cholinesterase Inhibitor | Rivastigmine (Exelon) Pused for pts that have PD w/dementia. |
Anticholinergic Drugs | benztropine (cogentin) & procyclidine (Kemadrin) given for severe tremors & rigidity. NOT for older adults- causes confusion, urinary retention, constipation, dry mouth, & blurred vision |
