antihistamine | prevent histamine response by blocking the H1 receptor sites, nasopharyngeal secretions are decreased |
first generation antihistamine | diphenhydramine, used for common cold, prevents motion sickness |
diphenhydramine teaching | take with food to avoid GI distress, use oral rinses, avoid alcohol or depressants because this causes drowsiness; avoid if pregnant |
second generation antihistamine | less able to pass BBB and cause sedation, minimal anticholinergic effect; loratidine |
intranasal glucocorticoids | anti-inflammatory which decreases swelling in nasal mucosa, decrease rhinnorhea, sneezing and congestion; a nasal steroid |
antitussive | act on cough control center and suppress cough reflex |
opioid antitussive | codeine, guaifenesin and codeine; used for severe cough causing inflamed airways and unable ADL |
guaifenesin | mucinex, must be taken with water |
non opioid antitussive | dextromethorpan (robitussin); used for generic cough |
expectorant | loosens bronchial secretions by reducing surface tension of secretions, eliminating by cough; increase fluid intake with these |
rebound nasal congestion | using longer than 3 days will elicit opposite effect |
anticholinergic bronchodilators | block the parasympathetic nerve reflexes that cause airways to constrict, allows passages to remain open |
bronchodilators | stimulate beta 2 adrenergic receptors in smooth muscle and cause bronchodilation to relieve bronchospasm |
bronchodilator short acting | rescue inhaler, relieves asthma attack |
glucocorticoids | decrease production of inflammatory mediators, decrease infiltration of inflammatory cells, decrease edema of airway, decrease mucus production |
oral decongestant contraindication | with cardiac conditions because they sympathetically stimulate BP and HR; do not use more than 3 days in a row |
side effects of glucocorticoids | hoarseness and dry mouth, hyperglycemia, euphoria, adrenal suppression, superinfections and osteoporosis |
bronchodilator side effects | cardiac, if there is a cardiac history, choose anticholinergic bronchodilator |
