skin with a defined area of persistent redness (in darker skin tones - red, blue or purple), Temp - hotter or colder than surrounding skin, Tissue consistency - may be firm or boggy, Pain or itch is what stage of a pressure ulcer stage I
tissue with partial thickness skin loss, superficial, looks like an abrasion, blister or shallow crater is what stage of an pressure ulcer stage II
tissue that has Full-thickness skin loss, Damage to or necrosis of sc tissue, and has a Deep crater is what stage of pressure ulcer stage III
tissue that has Full thickness skin loss with extensive damage, increased tissue necrosis, Damage to muscle, bone & supporting tissues is what stage of pressure ulcer stage IV
