Adverse reactions ataxia, agitation, coma, dizziness, drowsiness, diplopia, nystagmus, hypotension, gingival hyperplasia, nausea, rashes
Folic acid May decrease absorption of folic acid
PO Give with or after meals to minimize GI irritation
Pt teaching May cause drowsiness, dizziness. Notify provider if rash, slurred speak, yellow eyes, joint pain, fever, occurs
Therapeutic blood levels 10 to 20 mcg/mL
Signs of toxicity nystagmus, ataxia, confusion, nausea, slurred speech, dizziness
