IM Injections

What is IM injections? Medication injected into muscleRapid absorptionCan inject medications irritating to subcutaneous tissues
IM injections Always 90 degree angle. Needle gauge 20-22. Needle length 1 1/2-3 inches in adults (1 inch only in the Deltoid. Never inject no more than 3ml into any maid adult sites. Always wear gloves and always aspirate.
What factors are included when choosing a site? Muscle size, blood supply/circulation, viscosity of drug and irritating properties of the drug.
Dorsogleuteal Site (when is it used?) Not used for children under 3. Need to avoid large vessels and sciatic nerve. Pt can lay prone or side lying.
Dorsogleuteal Site Technique Identify landmarks: Posterior superior spine of Iliac crest, Greater trochanger of femur. Then traw imaginary line, site is lateral and superior to the line.
Ventrogleuteal Site No large nerves or vessels, less fat. Patients may lay supine or side lying.
Ventrogleuteal Site Technique Use left hand for right hip (make sure thumb points forward), Identify landmarks: place heal of hand on the greater trochanger, place index finger on anterior superior spine of iliac crest. Site is center of triangle formed.
Vastus Lateralis Anterior lateral thigh. No large blood vessels or nerves IF correctly identified.
Vastus Lateralis Technique Identify landmarks: one hand on the distal end of greater trochanger, and one hand on the lateral femoral condyle. Divide the thigh into even thirds. Site is the middle third of the anterior lateral thigh.
Deltoid No more than 1 cc in most adults. Close to radial nerve and artery.
Deltoid Technique Landmarks: 2 inches below the Acromium Process, Axilla. Form unsidedown U and site is within the U.
Z-Track IM Injections Used when medication is particularly irritating. Purpose-prevent medication from leaking into subcutaneous tissues. Examples- Vistaril (hydroxyzine) and Imferon (an iron preparation). Site is dorsogleuteal.
Z-Track IM Technique Filter needle used with ampules. Pull skin laterally and hold during entire injections. Insert needle, aspirate, inject med, cout to 10, withdraw needle-letting go of skin as needle is quickly withdrawn. Do not massage
