R/O MI
Question | Answer |
---|---|
Aspirin 325 mg EC | Salicylates, antipyretics , prophylaxis of transient ischemic attacks and MI. Reduces inflammation and decreases platelet aggregation. |
Nitroglycerin | antianginal, (nitrate) management of angina, treatment of MI, dilates coronary arteries increasing blood flow (vasodilation). Increases cardiac output. Side effects include, dizziness, headache, hypotension and tachycardia |
Morphine | opioid agonist, treatment for pain associated with MI (severe pain) Binds to opiate receptors in the CNS, alters the perception of and response to painful stimuli while producing generalized CNS depression. |
Morphine side effects | confusion, sedation, hypotension, constipation |
Nitroglycerin drip | prn and titrate for chest pain |
Metoprolol | Beta blocker Used for hypertension, angina pectoris, prevention of MI and decreased mortality in pt with recent MI, Blocks stimulation of beta1 adrenergic receptors, decreases BP and heart rate. IV= immediate onset, peak in 20 min and duration of 5-8 hr. |
Metoprolol side effects | fatigue, weakness, hypotension. |
Metoprolol unlabeled use | Unlabeled use=ventricular arrhythmias/tachycardia |
Quinapril | Ace inhibitor, management of hypertension and CHF, Improves survival and decreases development of overt CHF and MI. Block the conversion of angiotensin I to the vasoconstrictor angiotensin II |
Quinapril side effects | hypotension, cough, taste disturbances |
Captopril | Ace inhibitor, management of HTN, management of hypertension and CHF, Improves survival and decreases development of overt CHF and MI. Block the conversion of angiotensin I to the vasoconstrictor angiotensin II |
Captopril side effects | hypotension, dizziness, cough sob |
Diltiazem (Calcium Channel Blocker) | For HTN, angina, SVT and rapid ventricular rates in atrial flutter or fib. Inhibits transport of calcium into myocardial and vascular smooth muscle cells. Decreases bp, coronary vasodilation resulting in decreased frequency and severity of angina |
Diltiazem side effects | peripheral edema, cough, dyspnea, arrhythmias, hypotension |
Simvastatin | HS (lipid lowering agent) HMG-CoA reductase inhibitor, management of hypercholesterolemia. Decreases risk of MI, Lowers LDL and triglycerides |
Enoxaparin | Anticoagulant (antithrombotic) prevention of ischemic attacks , tx of ST segment elevation MI , prevents thrombus formation Caution if creatine clearance less than 30 |
Enoxaparin side effects | , risk of bleeding increased, urinary retention |
Heparin | Anticoagulant, prevention of thrombus formation |
Heparin side effects | anemia, thromycytopenia, pain at site, watch for bleeding |
Amiodarone | Antiarrhythmic Class III, management of life threatening arrhythmias, management of Ventricular fib/pulselessness, action=slows the sinus rate |
Amiodarone side effects | dizziness, fatigue, malaise, corneal microdepostis, brady, hypotension, N/V , ARDS, worsening of arrhythmias |
Dopamine | inotropic, vasopressor (adrenergics) tx of shock unresponsive to fluid replacement, increases renal perfusion in low doses, increases blood pressure and cardiac output, improves renal blood flow |
Dopamine side effects | arrhythmias, hypotension |
