RISK MANAGEMENT Process that centers on identification, analysis, treatment and evaluation of real and potential hazards. It is the scientific data to describe the form
RISK MANAGEMENT STRATEGIES Informed Consent, Unusual Occurrence Reports, Sentinel Events & Documentation.
INFORMED CONSENT To provide patients with enough information to enable them to make a rational decision regarding whether to undergo treatment. The focus is on a patients understanding the procedure not just signing the consent to have the procedure performed.
UNUSUAL OCCURRENCE REPORT Non judgmental, factual reports of the problem and its consequences. Records of an event that is a deviation from the standard and should be filed each time there is a deviation from the standard. It is considered an internal reporting mechanism
TEN KEY POINTS 0F UNUSUAL OCCURRENCE REPORT Admitting diagnosis; Date & time incidence occured; Pt. Rm#; Age of Pt.; Location of incident; Pt. condition before incident; Results of injury.
