Calcium Channel Blockers Prototype diltiazem (Cardizem) verapamil (Calan)
Calcium Channel Blockers MOA Block calcium influx into cardiac and vascular smooth muscle cells. Decreases contractility, increases vasodilation (decreasing bp), decreases conduction (decreasing heart rate).
Calcium Channel Blockers Adverse Effects Hypotension, palpitations, dizziness, bradycardia, heart failure. Constipation, nausea.
Calcium Channel Blockers Contraindications/Cautions Heart failure, Cardiac conduction problems (heart block), Bradycardia, Hypotension.
Calcium Channel Blockers Interactions Other antihypertensives, other drugs that lower heart rate.
Vasodilators Prototype sodium nitroprusside (Nipride, Nitropress)
Vasodilators MOA Directly relax arteriolar smooth muscle. Results in decreased systemic vascular response, decreased afterload and peripheral vasodilation.
Vasodilators Adverse Effects Refractory hypotension, rebound tachycardia, dizziness/lightheadedness. Cyanide toxicity with nitroprusside.
Vasodilators Indications Severe hypertension (hypertensive emergencies).
Vasodilators Cautions/Contraindications Coronary Artery Disease, Heart failure, peripheral vascular disease.
Vasodilator Interactions Any other hypertensives.
