What is Cushing Syndrome? Excess adrenocorticoid activity-hypersecretion of glucocorticoids from the adrenal cortex.
What might cause Cushing syndrome? Excess adrenocorticoid activity, adrenal, pituitary, or hypothalamus tumors.
Name s/s of Cushing syndrome. Moon face,truncal obesity,dowager’s hume,striae on abdomen,muscle atrophy,thinning skin,hirsutism in female,hyperpigmentation,EDEMA,POORWOUND HEALING, EASY BRUISING,HTN,SUSCEPTIBILITY TO MULTIPLE INFECTIONS,OSTEOPORAOSIS,PEPTIC ULCERS
What are some common lab findings with Cushing Syndrome? Hyperglycemia,hypernatremia,hypokalemia,decreased eosinophils and lymphocytes,increased plasma cortisol,increased urinary 17-hydroxycorticoids.
What are some nursing diagnoses for Cushing’s Syndrome? excess fluid vol. r/t,risk for infection r/t,disturbed body image r/t.
Discuss nursing plans/interventions for Cushing’s Syndrome. Protection from infection,wash hands,monitor for signs of infection(fever,oralCandida sp.,vaginalyeastinfections,adventitious lungs,skin lesion,elevated WBCs),good skin and perineal care, discuss weaningfrom steroids,I&Os,dailywt.PROVIDE ULCER PROPHYLAXIS
What safety measures may be initiated/taught for pt with Cushing’s syndrome? Bed close to floor,call light close,use side rails,clear walkways,wear shoes when ambulating.
What diet is good for Cushing’s syndrome? Low-sodium, foods containing Vit D and calcium.
What teaching should be done by RN to pt on steroids? Take steroids with meals to prevent GI irritation. Never skip doses. If N/V for longer than 12-24 hrs,contact dr.
What does cortisol do? F/E balance, metabolism of glucose and protein,suppression of inflammatory response to trauma,protective immune response to invasion of infectious agents, resistance to stress
What do mineral corticoids do? Electrolyte balance by promoting sodium retention and K+ secretion.
What is the difference between Cushing’s disease and Cushing’s syndrome? Disease –abnormally increased secretion (endogenous) of cortisol,caused by increased amounts of ACTH: syndrome-chronic and excessive production of cortisol or the administration of glucocorticoids in large doses for several wks or longer.
