Coronary Artery Dis
Ch. 29 Lemone
Coronary Artery Disease | Coronary Artery Disease |
---|---|
Coronary Artery Disease | Generic term for disorders of heart and blood vessels, caused by impaired blood flow to myocardium |
CAD | Cause is accumualtion of arthersclerotic plaque in coronary arteries, May be asymptomatic, cause of death for all u.s. ethnic groups |
CAD Risk Factors Non Modifiable | > 50% are 65 or older; 80% of deaths, Male, african american |
CAD Risk Factors Modifiable | Hypertension, Diabetes mellitus, Hyperlipidemia, Elevated LDL, HDL above 35, reduce risk of CHD, elevated homocystine levels, Metabolic syndrome, Smoking, Obesity |
Artherosclerosis | Plaque formation, affecting the medial and intima of the medium arteries |
Myocardial Ischemia | Myocardial cells are ischemic when oxygen supply inadequate for metabolic demands and is dependent upon. |
Myocardial Ischemia process | myocardial cells have limited supplies of adenosine triphosphate for energy storage; store depleted contractibility affected |
Myocardial Ishemia Cell Metabolism | switches to anaerobic and produces lactic acid, after 20 min. aerobic metab and contactibility restored and cellular repair begins. |
Coronary Heart Disease 2 Catagories | Chronic ischemic heart diseas and Acute coronary syndrome. |
Collaborative care for Cardio Vascular Disease | Aggresive Risk Factor Management 1. Slow down arthersclerotic process 2. Maintain myocardial perfusion |
CVD Diagnostic Test | ECG,Exercise Stress Tests, Coronary Angiogram, EBCT, PTCA,CABG |
CVD (Risk Factor Management) | 1. Smoking 2. Diet 3. Exercise 4. Hypertension 5. Diabetes 6. Medications 7. Health Promotion |
Diet for Coronary Heart Disease | Avoid Saturated Fat, Include nonfat dairy products, fish, poultry as protien sources; us olive, canola, peanut oil, Increase fiber, Folic acid, Bit. B6 and B12, Moderate alcohol. Overweight encourage to lose weight. Excercise 5 to 6 times for 30 min. |
Medications for CHD | Cholesterol lowering drugs; Lower LDL, Bile acid sequestrants, Nicotinic acid, Fibrates, Low dose aspirin, ACE inhibitors |
Nursing Diagnosis | 1. Imbalanced Nutrition; More than body requirements, 2. Ineffective Health Maintnance; Interventions to lower or remove risk factors. |
