Metformin (Glucophage) or XR 1-3 times with meals decrease glucose release from liver, decrease absorption in SI, improves sensitivity GI bloating, N, D, monitor kidney and liver fxn, should have good creatinine clearance, withhold 48 hrs pre-contrast dye testing
Pioglitazone (Actos) once daily with or without meals reduces insulin resistance, takes 4-6 weeks fluid retentn, wt gain, ovulation and decreased oral contracept, monitor liver fxn
Acarbose (Precose) 3 times with first bite of meals decrease digest and absorp of carbs Diarrhea, gas, pain, avoid with IBD, TREAT hypoglycemia with MILK/GLUCOSE ONLY, may be intolerable
Exenatide (Byetta) 2 times SQ, up to 60 min pre-brkfst, dinner slows gastric emptying, stim. insulin, suppr. glucagon, suppre. appetite hypogly, nausea, improves control with metformin or glipizide(glucotrol)
Pramlintide (Symlin) Immed pre major meals complements insulin, slows gastric emptying, suppress glucagon, suppress appetite, acts like amylin N, V, no GI prob people
