GERD | Reflux of gastric secretions from the stomach to the esophagus. |
Predisposing factors | Hiatal hernia, weak lower LES, alcohol, decreased gastric emptying and decreased clearance. |
Etiology | Acidic gastric secretions reflux causes irritation and inflammation. |
S & SX: | Heartburn. This occurs more than once a week or at night. Regurgitation is common. It is described as bitter and sour liquid coming into the mouth. GI symptoms include: bloating, N & V. |
Complications: | Esophagitis, Barret's esophagus, respiratory irritation/problems and dental problems. |
DX: | Barium studies; biopsy; esophageal pressure measures & pH monitoring. |
Care: Lifestyle changes | Avoid foods that trigger it. Stop smoking. |
Care: Nutrition therapy | Decrease high fat intake, avoid milk at night, avoid foods/fluids with caffiene and late night snacking. |
Drugs: | H2 receptor blockers, Antacids, proton pump inhibitors, sucralfate and prokinetic drugs. |
Prevention: | Avoid factors that trigger it. For example, stop smoking & avoid acid producing foods. Elevate the HOB @ 30 degrees. Do not lie down after 2 to 3 hours after eating. Drug therapy: evaluate the effectiveness and observe for side effects. |
