Hiatal Hernia

Hiatal Hernia | A herniation of the stomach into the esophagus through an opening in the diaphragm. |
Sliding Hernia | Stomach slides into thoracic cavity when supine and goes back when upright. |
Paraesophageal or rolling | Fundas and greater curvature of the stomach rolls up via the diaphragm. |
Multiple causes | Weakening of the muscles in the diaphragm. Increased intraabdominal pressure. Increasing age, trauma, poor nutrition, and prolonged recumbent position. |
S & SX | Similar to GERD. Heartburn, dysphagia, reflux & pain/discomfort. |
Complications: | GERD, hemorrhage, stenosis of the esophagus, ulcerations of the herinated portion of the stomach, strangulation of the hernia and regurgitation. |
DX: | Barium swallow & endoscopy. |
Care: Conservative Therapy | Lifestyle modifications, elevation of the HOB at 30 degress, antacids and H2-Receptors & weight reduction program. |
