Hiatal Hernia

Hiatal Hernia A herniation of the stomach into the esophagus through an opening in the diaphragm.
Sliding Hernia Stomach slides into thoracic cavity when supine and goes back when upright.
Paraesophageal or rolling Fundas and greater curvature of the stomach rolls up via the diaphragm.
Multiple causes Weakening of the muscles in the diaphragm. Increased intraabdominal pressure. Increasing age, trauma, poor nutrition, and prolonged recumbent position.
S & SX Similar to GERD. Heartburn, dysphagia, reflux & pain/discomfort.
Complications: GERD, hemorrhage, stenosis of the esophagus, ulcerations of the herinated portion of the stomach, strangulation of the hernia and regurgitation.
DX: Barium swallow & endoscopy.
Care: Conservative Therapy Lifestyle modifications, elevation of the HOB at 30 degress, antacids and H2-Receptors & weight reduction program.
