Colorectal Cancer

Colorectal Cancer 2nd most common cancer in Canada. Over the age 40.
Causes: Genetics, Age over 40, high calorie/fat Western diet.
Patho: Adeocarinoma is the most common type which arises from polyps. Tumors spread via the walls of the intestine and into the lymphatic system. Tumors commonly spread to the liver because the venous blood flow from the colorectal tumor is via the portal vein
Clinical Manifestation Nonspecific. May appear until advanced cancer. Cancer to the right side gives arise to different S & SX to cancer to the left side. Rectal bleeding is the most common type of symptom.
Diagnostic studies History and physical examination. Digital rectal examination. Colonoscopy. Sigmoidoscopy. Barium enema. Endorectal ultrasound, CT scan, Occult blood test, liver function tests & CBC.
Collaborative Therapy: Surgery Right & left hemicolectomy, ab-perinal resection & laparoscopic colectomy.
Care: Radiation In combination with surgery (colon resection), pallative care to reduce tumors and symptom relief.
Care: Chemotherapy Used when the client has positive lymph nodes at the time of surgery. In combination with surgery (colon resection) and primary treament for nonsurgery cancer.
Health Promotion Fecal occult blood testing at the age of 50 every 1 or 2 years. Positive findings should follow up with colonoscopy or barium enema. Screening for high risk should begin before the age 50.
