Sodium Bicarb < 3 mo = 1 mEq/kg of 4.2% > 3 mo = 1 mEq/kg of 8.4%
Lidocaine VF, pVT 1 mg/kg repeat in 3-5 min max 3 mg/kg
Lidocaine VT w/ pulse 1 mg/kg every 5-10 min pkg: 100 mg/5 ml
Lidocaine IV drip 20-50 mcg/kg/min
Amiodarone VF, pVT 5 mg/kg IV/IO max 15 mg/kg
Amiodarone SVT, Ventricular arrhythmias 5 mg/kg over 20-60 minutes
Atropine OGP begin with .05 mg/kg repeat with .05 mg/kg 5-10 min
Adenosine SVT/PSVT 1st = .1 mg/kg to max 6 mg 2nd = .2 mg/kg to max 12 mg may repeat 2nd dose 1 more X
Epinephrine (1:1,000) Asthma-severe distress/ Allergic reaction .01 mg/kg IM/SQ may repeat in 20 min - max of .3 mg
Epinephrine (1:1,000) Anaphalaxis < 30 kg: 0.01 mg/kg IM /SQ anterolateral thigh; repeat every 5 to 10 min prn - Max 0.3 mg > 30 kg: 0.3 to 0.5 mg IM /SQ anterolateral thigh; repeat every 5 to 10 minutes prn - Max 0.5 mg
Epinephrine (1:10,000) Pulseless Arrest (PEA, VF, pVT, Asystole, or HR <60 in neonates and Bradycardia) .01 mg/kg IV/IO
Epinephrine (1:10,000) Anaphalaxis - Neonate: 0.01 mg/kg IV slowly once - Infant: 0.05 mg IV slowly once; may repeat at 20 to 30 min intervals prn
Calcium Chloride 2-4 mg/kg pkg: 1 g in prefilled 10 ml, 10% solution
Phenergan .5-1.0 mg/kg PR
Mannitol To decrease ICP .5 g/kg slow over 30-60 min 100 g/10mL must use inline filter, can crystallize
Succinylcholine 1-2 mg/kg IV/IM/IO pkg: 200 mg/10mL
Veccuronium .1 mg/kg to a max single dose of 10 mg/kg pkg: 10 mg/10mL
Diazepam seizures .25 mg/kg IVP slow over 2 min, max 1 mg/kg titrate to effect
Midazolam .05 mg/kg or 3 mg IN pkg: 1 mg/kg or 5 mg/mL
Fentanyl > 2 yrs = 2 mcg/kg slow over 1 min
Morphine Pain .1 mg/kg repeat q 5 min prn pkg: 2 mg/mL ampule
Dextrose .5 g/kg of 25% ( take D-50 empty 25 cc's and refill w/ NS) Neonates .5 g/kg of D-10
Naloxone < 5 yrs .4 - 2 mg - may repeat after 3 min if no change seen. > 5 yrs .1 mg/kg pkg: 2 mg/mL
Glucagon Hypoglycemia 1 unit IM may repeat q 3 min.
Glucagon Beta Blocker OD 1-3 units – may repeat q 20 min. pkg: 1 unit with solute
Activated Charcoal 1 g/kg ( typically 25 g) PO pkg: 25 g/125mL
Diphenhydramine 1 mg/kg (max single dose 50 mg) pkg: 50 mg/1mL
Ipratropium Bromide .15 -. 25 mg nebulizer (One time only) pkg: .5 mg/3mL
Terbutaline .01 mg/kg SQ pkg: 1 mg/1mL
