Adenosine (Adenocard) Adult: 6mg rapid IV push, 20ml bolus. After 1-2 min 12mg rapid IV push, 20ml bolus. Max 18mg Pediatric: 0.1mg/kg rapid IV push, 20ml bolus. After 1-2 min 0.2mg/kg rapid IV push, 20 ml bolus.
Albuterol Adult: 2.5mg mixed with 2.5ml normal saline Pediatric: 2.5mg in 6ml normal saline
Amiodarone Adult: 300mg IV push min 14ml NS; repeat dose: 150mg IV push in 7ml NS Pediatric: 5mg/kg slow IV push over 1-2 minutes in 10ml NS VT w/ Pulse: Adult: 150mg in 100ml NS infused over 10 minutes.
Aspirin 81mg - 324mg PO
Atropine Sulfate Adult: 0.5mg IV every 3-5 min as needed to max of 3mg. Pediatric: 0.02mg/kg IV (min. 0.1mg; max 1mg; single dose)
Diltiazem HCL (Cardizem) Adult: 0.25mg/kg IV over 2 minutes. If inadequate response, repeat at 0.35mg/kg IV over 2 minutes. Pediatric: NOT RECOMMENDED
Epinephrine Adult: Allergic reactions/asthma: 03.-0.5mg IM (0.3-0.5ml 1:1,000) Anaphylaxis: 0.3-0.5mg IV (3-5ml 1:10,000) Cardiac: 1mg IV push every 3-5 min. Pediatric: 0.01mg/kg IM max of 0.5mg (1:1,000) Cardiac: 0.01mg/kg IV (1:10,000)
Hydrocortisone/Methylprednisolone Hydrocortisone: Adult: 100mg IV; Pedi: 2mg/kg IV max of 100mg Methylprednisolone: Adult: 125mg IV; Pedi: 2mg/kg IV max of 125mg
Ipratropium Bromide (Atrovent) Adult: 0.5 mg via Neb. usually mixed with 2.5mg albuterol Pediatric: 250 mcg via Neb, may mix with albuterol
Magnesium Sulfate Adult: Seizures assoc. w/ preg: 1-4g IV over 10 min Torsades/Refractory VF/VT: 1-2g IV over 1-2 min Bronchospasm: 2-4g IV over 5 min Pedi: Bronchospasm(severe): 25-50mg/kg IV over 10 min, usually mixed in 50-100cc NS
Metoprolol Adult: 5mg over 1-2min slow IV push Pediatric: Not Recommended
Nitroglycerin Adult: Tablets: 0.3 mg SL; may repeat in 3-5 min to max 3 doses. NTG Spray: 0.4mg SL; 1-2 sprays
