Adenosine 6mg rapid IVP, followed by 20mL flush. May repeat at 12 mg
Albuterol 2.5mg/3mL neb
Aminophylline 5mg/kg drip over 20-30 minutes
Amiodarone Dead: 300mg IVP, may repeat at 150mg. Alive: 150mg over 10 minutes
Atropine (symptomatic bradycardia) 0.5-1mg IVP Q5. Max dose 3 mg
Atropine (organophosphate) 1-2mg IV/IM per MD orders Q5 until symptoms reside
Calcium Chloride 500-1000mg slow IVP
Calcium Gluconate 10-30 mL slow IVP
Clopidrogel 300mg PO
Diazepam 4mg IVP. Repeat at 2mg Q5
Diltiazem (push dose) 0.25 mg/kg
Diltiazem (drip) 5-10mg/hr
Dobutamine 2-20 mcg/kg/min
Dopamine (beta) 2-10 mcg/kg/min
Dopamine (alpha) 11-20 mcg/kg/min
Dexamethasone 10-20 mg slow IVP
Epi (drip) Mix 1mg:250mL (4mcg/mL) Drip 1mcg/min
Epi 1:10 1mg Q 3min IVP
Etomidate 0.3mg/kg IVP
Fentanyl 50-75 mcg loading dose, repeat Q20. Max 200mcg
Furosemide 20-80 mg IVP
Flumazenil 0.2-0.5 mg over 15 seconds. Max 3 mg
Glucagon 1-2 mg IM
Haloperidol 5-10 mg IV/IM
Hydromorphone 1-2 mg Q20-30 minutes. Max 5mg
Ipratropium 500mcg/2.5mL neb
Isoproterenol Mix 1mg:500mL (2mcg/mL). Drip 2-10 mcg/min titrated to effect
Keterolac 30mg IV; 30-60mg IM
Ketamine (sedation) 1-2 mg/kg IVP
Ketamine (pain) 0.1-0.3mg/kg IVP
Labetelol 10mg IVP
Levalbuterol 0.63-1.25 mg/3cc neb
Lorazepam 2-4 mg IV/IM Q5
Lidocaine (anti-arrhythmic) 1-1.5 mg/kg. Repeat at half. Max 3mg/kg
Magnesium (eclampsia) 1-2g IV over 1-5 minutes
Methylprednisolone 125mg IVP
Metoclopramide 5-10 mg IV/IM
Metoprolol 5mg slow IVP Q5. Max 15mg
Midazolam 0.5-2mg IVP, 5mg IM, 1-2mg NA. Max dose 5mg
Morphine 2-4mg IVP (0.1mg/kg)
Naloxone 0.5-2 mg Q3-5 IVP, IM, NA
Norcuron 0.1 mg/kg
Norepinepherine Mix 4mg in 250mL (1600mcg/mL). Drip 0.5-30 mcg/min)
Ondansetron 4mg IV/IM
Oxytocin (IM) 3-10u IM
Oxytocin (Drip) Mix 10-20u in 500-1000mL, titrate to effect
Procainamide (loading dose) 100mg IVP or 20mg/min drip
Procainamide (maintenance drip) 1-4mg/min. Max dose 17mg/kg
Promethazine 12.5-25 mg IVP/IM
Prednisone 40-60mg PO
Propanolol 0.1 mg/kg IVP
Protopam 600mg IV/IM. Always give with atropine
Racemic Epi Mix 0.5mL in 4mL saline and neb. Patients must be admitted for obs
Sodium Bicarbonate 50meq IVP. May repeat at 0.5 amp Q10
Sodium Nitroprusside 0.1-0.5 mcg/kg/min. Must be given in D5W
Succinylcholine 1.5mg/kg IV/IO
Thiamine 100 mg IV/IM
Vasopressin 40u IV
Ziprasidone 40u IM
