Medications

Adenosine Dosage: Pedi >50kg Adult dosing.
Adenosine Dosage: Pedi <50kg Initial dose 0.1mg/kg IV, IO (max dose 6mg) followed by >/= 5ml saline flush; may repeat at 0.2mg/kg (max dose 12mg).
Albuterol Dosage: MDI: Pedi < 4yrs <4yrs Adinister by nebulization.
Albuterol Dosage: Nebulizer Pedi > 12yrs 0.15mg/kg every 20 minutes for maximum of 3 doses. Alternatively, continuous nebulization @ 0.5 mg/kg/hr can be delivered to children > 12yrs.
Amiodarone Indications • Treatment of V-fib or V-Tach, Chemical cardioversion of A-fib or A-flutter
Amiodarone Dosage: Pedi Pulseless V-tach or V-fib cardiac arrest: 5 mg/kg IV/IO push. May repeat at 5 mg/kg IV/IO push to a max dose of 15 mg/kg IV/IO push. Max single dose of 300mg
Atropine Indications Asystole and PEA, Increase HR in unstable bradycardia, Improve conduction in 2nd and 3rd degree block, Insecticide exposure and nerve gases, Asthma, Counteract excessive vagal influences in RSI
Atropine Dosage: Pedi: 0.02 mg/kg IV/IO push
Epinephrine Class Sympathomimetic
Epinephrine Indications PEA, Asystole, Symptomatic bradycardia refractory to atropine, pacing, and dopamine, Severe allergic reactions and anaphylaxis, V-Fib, Acute asthma attacks, Hypotension not responsive to dopamine
Epinephrine Dosage: Pedi w/ cardiac arrest and bradycardia: .01 mg/kg 1:10,000 IV/IO push q 3-5 mins (ETT at 0.1 mg/kg 1:1000)
Epinephrine Dosage: Pedi Allergic/Anaphalaxis 0.01 mg/kg 1:1000 SQ/IM
Promethazine Class Antiemetic, antihistamine
Promethazine Indications Nausea/vomiting
Promethazine Dosage: Pedi >2 yrs: 0.25-1mg/kg IV, IO, IM (max rate of iv, IO administration is 25mg/min)
Zofran, also known known as Ondansetron
Zofran Class Antiemetic
Zofran Indications Nausea/vomiting
Zofran Dosage: Pedi: .15mg/kg every 4 hrs w/ max dose of 4mg.
Dextrose Class Hyperglycemic
Dextrose Indications hyperglycemia and hyperkalemia
Dextrose Dosage: Pedi w/ hyperkalemia: 0.5-1g/kg IV, IO.
Dextrose Dosage: Pedi w/ hyperglycemia: > 2yrs: 2ml/kg of dextrose 50%.
Dextrose Dosage: Pedi w/ hyperglycemia; < 2yrs: 2-4ml/kg of dextrose 10%.
Fentanyl, also known as Sublimaze
Fentanyl Class Narcotic analgesic; schedule C-II.
Fentanyl Indications Pain
Fentanyl Dosage: Pedi: 1-2mcg/kg IM or slow IV, IO (admin'd over 1-2 min).
Morphine Sulfate Class Opiate agonist, schedule C-II.
Morphine Sulfate Indications: Moderate to severe pain, including chest pain associated with ACS, CHF, and pulmonary edema.
Morphine Sulfate Dosage: Pedi w/ pain: 6months-12 years; 0.05-0.2mg/kg IV, IO, IM, sub-Q. Younger than 6 months: 0.03-0.05mg/kg IV, IO, IM, sub-Q.
Morphine Sulfate Dosage: Pedi w/ Chest pain associated with acute coronary syndrome, CHF, and pulmonary edema (Small doses and reevaluate patient. Large doses may lead to respiratory depression and worsen the patient’s hypoxia): 0.1-0.2mg/kg/dose IV, IO.
Racemic Epinephrine/Racepinephrine, also known as microNefrin, S2
Racemic Epinephrine Class Bronchodilator, adrenergic agent.
Racemic Epinephrine Indications Bronchial asthma, croup.
Racemic Epinephrine Dosage: Pedi > 4yrs: Same as adult dosing.
Racemic Epinephrine Dosage: Pedi < 4yrs: Safe effective has not been demonstrated.
