The "S" in the SOAP Method stands for... | SUBJECTIVE: The problem in the patients own words. Includes the duration of the problem, the quality of the problem, and any exacerbating or relieving factors for that problem. |
The "O" in the SOAP Method stands for... | OBJECTIVE: Collecting objective data. Compromises the patient's physical exam, any laboratory findings, and imaging studies performed at the visit. |
The "A" in the SOAP Method stands for... | ASSESSMENT: Formulation of a logical analysis. Could be a diagnosis, and identification of a problem, or a list of possibilities for the diagnosis, which is known as a differential diagnosis. |
The "P" in the SOAP Method stands for... | PLAN: The formulation of a course of action consistent with the assessment. Could be a treatment with medicine or a procedure. Could also consist of collecting further data to help arrive at a more accurate diagnosis. |
